Continental Healthcare Services (CHCS) is a registered provider with the Department of Veterans’ Affairs (DVA), providing dedicated nursing, personal care and community support services to veterans.
Our experienced team works collaboratively with veterans, their families and healthcare professionals to provide safe, coordinated and person-centred care that supports independence, dignity and wellbeing.
DVA Nursing Services
Our experienced nursing team provides professional care tailored to each individual’s health needs, circumstances and goals.
Our nursing services include:
- Comprehensive health assessments
- Medication management and administration
- Wound care and pressure injury management
- Chronic disease management, including diabetes, cardiac and respiratory conditions
- Palliative and end-of-life nursing care
- Post-hospital discharge and recovery support
- Continence assessments and care
- Pain management and symptom monitoring
- Catheter and stoma care
- Allied health coordination and care planning
Personal & Daily Living Support
We provide practical day-to-day assistance to help veterans maintain their independence and quality of life at home.
Support can include:
- Personal care, including showering, dressing and grooming
- Mobility and transfer support
- Meal preparation and nutrition support
- Domestic assistance, including cleaning, laundry and light household tasks
Community & Wellbeing Support
Our services extend beyond the home, helping veterans remain connected with their families, communities and the activities that matter to them.
Support can include:
- Social support and companionship
- Community participation and outings
- Transport assistance to medical and community appointments
- Support to attend veteran, family and community events
Mental Health & Emotional Wellbeing
Our team provides compassionate, veteran-centred support focused on mental health and emotional wellbeing.
This can include:
- Monitoring mental health and wellbeing
- Trauma-informed and veteran-centred care
- Support for PTSD, anxiety and depression in collaboration with clinicians
- Family and carer support
Personal & Daily Living Care
We understand that care needs can change over time. CHCS provides flexible support that can be tailored to the individual and their circumstances.
Services include:
- In-home support
- Short-term care following illness or hospitalisation
- Flexible care tailored to changing health needs
Care Coordination & Advocacy
We can help coordinate care across different healthcare providers and services, making it easier for veterans and their families to access the support they need.
Our team can assist with:
- Liaising with GPs, hospitals, allied health professionals and DVA
- Care planning and service coordination
- Support navigating DVA services and entitlements
Experienced & Compassionate Care
CHCS was established to provide trusted, high-quality healthcare services tailored to individual needs.
Our team includes experienced registered nurses, enrolled nurses and support workers, with care delivered around the needs of each individual.
Our Founder and Director brings more than 25 years of nursing experience across intensive care, coronary care, theatre recovery, renal and dialysis, mental health, and medical-surgical settings.
